Association of COVID-19 coinfection with increased mortality among patients with Pseudomonas aeruginosa bloodstream infection in the Veterans Health Administration system

Objective:
Pseudomonas aeruginosa bloodstream infection (PA-BSI) and COVID-19 are independently associated with high mortality. We sought to demonstrate the impact of COVID-19 coinfection on patients with PA-BSI.
Design:Retrospective cohort study.
Setting:Veterans Health Administration.
Patients:Hospitalized patients with PA-BSI in pre-COVID-19 (January 2009 to December 2019) and COVID-19 (January 2020 to June 2022) periods. Patients in the COVID-19 period were further stratified by the presence or absence of concomitant COVID-19 infection.
Methods:We characterized trends in resistance, treatment, and mortality over the study period. Multivariable logistic regression and modified Poisson analyses were used to determine the association between COVID-19 and mortality among patients with PA-BSI. Additional predictors included demographics, comorbidities, disease severity, antimicrobial susceptibility, and treatment.
Results:A total of 6,714 patients with PA-BSI were identified. Throughout the study period, PA resistance rates decreased. Mortality decreased during the pre-COVID-19 period and increased during the COVID-19 period. Mortality was not significantly different between pre-COVID-19 (24.5%, 95% confidence interval [CI] 23.3–28.6) and COVID-19 period/COVID-negative (26.0%, 95% CI 23.5–28.6) patients, but it was significantly higher in COVID-19 period/COVID-positive patients (47.2%, 35.3–59.3). In the modified Poisson analysis, COVID-19 coinfection was associated with higher mortality (relative risk 1.44, 95% CI 1.01–2.06). Higher Charlson Comorbidity Index, higher modified Acute Physiology and Chronic Health Evaluation score, and no targeted PA-BSI treatment within 48 h were also predictors of higher mortality.
Conclusions:Higher mortality was observed in patients with COVID-19 coinfection among patients with PA-BSI. Future studies should explore this relationship in other settings and investigate potential SARS-CoV-2 and PA synergy.

A brief overview of the research activities at the Thermionic Energy Conversion (TEC) Center is given. The goal is to achieve direct thermal to electric energy conversion with >20% efficiency and >1W/cm2 power density at a hot side temperature of 300–650C. Thermionic emission in both vacuum and solid-state devices is investigated. In the case of solid-state devices, hot electron filtering using heterostructure barriers is used to increase the thermoelectric power factor. In order to study electron transport above the barriers and lateral momentum conservation in thermionic emission process, the current-voltage characteristic of ballistic transistor structures is investigated. Embedded ErAs nanoparticles and metal/semiconductor multilayers are used to reduce the lattice thermal conductivity. Cross-plane thermoelectric properties and the effective ZT of the thin film are analyzed using the transient Harman technique. Integrated circuit fabrication techniques are used to transfer the n- and p-type thin films on AlN substrates and make power generation modules with hundreds of thin film elements. For vacuum devices, nitrogen-doped diamond and carbon nanotubes are studied for emitters. Sb-doped highly oriented diamond and low electron affinity AlGaN are investigated for collectors. Work functions below 1.6eV and vacuum thermionic power generation at temperatures below 700C have been demonstrated.

Organic chromophores have been exploited for a wide range of discrete optical and electronic functions as well as a growing number of combined opto-electronic functions. We are pursuing development of organic and polymer materials for a range of applications that require properties including liquid crystallinity, second order optical nonlinearity, photorefractivity and, more recently, special nonlinear optical behavior involving molecular chirality.

Syntactic magnesium foams which consist of thin-walled hollow alumina spheres embedded in a magnesium matrix were fabricated by infiltrating a three-dimensional array of hollow spheres with a magnesium melt by using a gas pressure-assisted casting technique.
The resulting composite contains closed cells of homogeneous and isotropic morphology. The densities of the syntactic magnesium foams were between 1.0 and 1.4 g/cm3. The densities were controlled by variations in the bulk density of the hollow spheres with the volume fraction of spheres kept constant at approximately 63 %.
Compressive deformation characteristics of the composites were evaluated with respect to the influence of matrix strength and sphere wall thickness on characteristic variables such as compressive strength, plateau stress and energy absorption efficiency. Differences in the strength of the magnesium-based matrix materials investigated (cp-Mg, AM20, AM50, AZ91) had little influence on the compressive strength of the syntactic foam. However, an increasing relative wall thickness of the hollow ceramic spheres led to a significant strength enhancement. In all cases the ratio between compressive and plateau strength rose with increasing composite strength resulting in decreasing energy absorption efficiency.

Let V be a four-dimensional complex symplectic vector space. This paper classifies those connected linear algebraic subgroups of the symplectic group Sp(V) that admit two independent rational invariants. As an application we show the non integrability of a three degree of freedom Hamiltonian system.

An extensive psychometric test battery was administered to 125 children with a reading disability, to their parents and siblings, and to members of 125 matched control families (N = 1,044). In addition to expected differences in spelling and reading, probands obtained significantly (P ≤ · 01) lower scores than controls on tests of other cognitive abilities. Manifold deficits were also found in siblings and parents of probands, conclusively demonstrating the familial (genetic and/or common-family environmental) nature of the disorder.

A number of anthropometric measurements were made on Southern Chinese women. These were 317 schizophrenics, 44 with affective disorder, 47 neurotics and normal controls. All groups and sub-groups compared were matched for mean age and socio-economic status. It was found that the intercorrelations between body build, personality and type of mental illness were on the whole similar to those reported for Caucasians.

In Parts I and II of this paper ((4), (5)) we studied the ‘spectral asymmetry’ of certain elliptic self-adjoint operators arising in Riemannian geometry. More precisely, for any elliptic self-adjoint operator A on a compact manifold we defined
where λ runs over the eigenvalues of A. For the particular operators of interest in Riemannian geometry we showed that ηA(s) had an analytic continuation to the whole complex s-plane, with simple poles, and that s = 0 was not a pole. The real number ηA(0), which is a measure of ‘spectral asymmetry’, was studied in detail particularly in relation to representations of the fundamental group.

1. Introduction. The main purpose of this paper is to present a generalization of Hirzebruch's signature theorem for the case of manifolds with boundary. Our result is in the framework of Riemannian geometry and can be viewed as analogous to the Gauss–Bonnet theorem for manifolds with boundary, although there is a very significant difference between the two cases which is, in a sense, the central topic of the paper. To explain this difference let us begin by recalling that the classical Gauss–Bonnet theorem for a surface X with boundary Y asserts that the Euler characteristic E(X) is given by a formula:
where K is the Gauss curvature of X and σ is the geodesic curvature of Y in X. In particular if, near the boundary, X is isometric to the product Y x R+, the boundary integral in (1.1) vanishes and the formula is the same as for closed surfaces. Similar remarks hold in higher dimensions. Now if X is a closed oriented Riemannian manifold of dimension 4, there is another formula relating cohomological invariants with curvature in addition to the Gauss–Bonnet formula. This expresses the signature of the quadratic form on H2(X, R) by an integral formula
where p1 is the differential 4-form representing the first Pontrjagin class and is given in terms of the curvature matrix R by p1 = (2π)−2Tr R2. It is natural to ask if (1.2) continues to hold for manifolds with boundary, provided the metric is a product near the boundary. Simple examples show that this is false, so that in general
